Voluntary Self-Identification
The Company is subject to certain governmental recordkeeping and reporting requirements, and as such, needs to track certain information relating to its employees. In an effort to comply with these requirements, the Company invites you to complete this Voluntary Self-Identification form.
Providing this information is STRICTLY VOLUNTARY. Your cooperation is appreciated, and failure to provide this information will not subject you to any adverse personnel action. However, because the Company is required by federal regulations to maintain this data, in the absence of data provided by you, the Company will use visual observation to record each employee's gender and race/ethnicity. Veteran status and disability status are recorded only from the information you choose to provide, and are never assumed or observed. This information will be kept confidential, used only in accordance with applicable laws and regulations, and may be used in aggregate form for other reporting purposes. No personally identifiable or sensitive personal information will be shared.
